Program Eligibility & How to Apply
Criteria for Eligibility
To be eligible for services at PEP, we require a copy of the individual’s IEP and/or previous program experiences, medical history, and behavioral history. We consider all applicants on a case-by-case basis.
Application Process
1. Complete and submit the PEP Member Application. Applications may be emailed to pephhistaff@gmail.com or dropped off in-person at PEP.
2. Our Program Administrator will contact you to schedule a tour for you and your prospective member and may request further documentation.

Program Fees & Payment Options
Medicaid Waivers or Scholarships
PEP accepts waivers for eligible members. The waivers accepted by PEP are the Community Supports (CS) waiver and the Intellectual Disability and Related Disabilities (ID/RD) waiver.
If you currently do not have a waiver, you may inquire with a member of the PEP Administration team about available resources to assist your family. If you wish to apply for Medicaid, the Medicaid Form 3400 and Medicaid Form 3400-A will need to be filled out.
We also offer scholarships when funding is available; however, scholarships are limited and not guaranteed.
Daily Fees
- Full day with inbound/outbound bus transportation: $75 per day
- Full day with one-way bus transportation (i.e., inbound OR outbound): $70 per day
- Full day without bus transportation: $65 per day
- Half day with one-way bus transportation (i.e., inbound OR outbound): $55 per day
- Half day without bus transportation: $45 per day
Private Pay
If you do not have a PEP-approved Medicaid waiver, that is considered “Private Pay,” and a person without a Medicaid waiver will most likely need to use their own funds to cover costs until a different funding source is secured. Monthly fees at PEP are determined by the number of days a member attends programming and transportation needs.
At the beginning of each month, parents/guardians receive an emailed invoice that lists the services and total costs accrued for the previous month.
